Oak Hills Terrace offers Memory Care services for those living with Alzheimer’s’ and other memory loss conditions. Our memory care encourages seniors with memory loss to live to their full potential and live each day purposefully. At Oak Hills Terrace, we prioritize fostering residents’ creativity, independence, and choice. Our team helps those in memory care by giving them the tools they need to feel at home at Oak Hills Terrace.

Residents of our Memory Care can take advantage of all the services and amenities Oak Hills Terrace offers. All classes and events are tailored to their unique needs and occur in secured areas. Amenities are also available for memory care residents, including housekeeping, laundry services, and more.

The friendly and compassionate staff at Oak Hills Terrace is proud to offer our Memory Care residents our award-winning Spark® program.  Imagine a program designed for seniors that strives to help them reach their full potential and allows them to spend their days with a joyful purpose. Frontier Management’s award-winning Spark® program is precisely that!

Spark® is a research-based program inspired by the teachings and research of Maria Montessori and Dr. Cameron Camp, which focuses on giving residents meaningful ways to feel needed and be productive. We use tailor-fit assessment tools to evaluate each resident’s capabilities and strengths so that the program can best meet their needs.

The Four Pillars of the Frontier Spark® Program 

The effectiveness and success of our Spark® program for Memory Care residents is based on 4 foundational pillars.

• Social: We ensure that each resident has a role in the community that allows them to interact with others.
• Skills Focused: Our Montessori Assessment Tool uncovers remaining skills often missed by traditional evaluations.
• Procedural Memory: Skills can be learned and even improved through repetitive “procedural learning,” which allows residents to perform routine activities with less concerted effort.
• Progression & Goal Setting: Residents who work on projects or learn new things over a period of time with a set goal in mind feel more engaged and successful.

Health & Wellness Services

Whether you or your loved one requires some help with the activities of daily living (ADLs) or is living with the effects of dementia or Alzheimer’s disease, our dedicated staff works hard to deliver high-quality health and wellness services that are compassionate, safe, and resident-centered. Our services are designed to meet our residents’ physical, emotional, and spiritual needs 24 hours a day, seven days a week. We offer enriching brain games and activities, exciting social programs, wellness and fitness classes, and more.

Before a resident moves into the community, customized assessments are completed to create individual care plans. These care plans are updated biannually or earlier if needed. Care plans are overseen by the Executive Director, staff, Resident Care Coordinators, and RNs. In addition to the Medical Director and highly trained nursing staff, our community is staffed by a Certified Activity Director, Certified Assisted Living Manager, Certified Food Managers, and medical technicians.

Accommodations & Amenities

Oak Hills Terrace features fine living and dining accommodations, beautiful courtyards with walking paths, seating, and areas for playing outdoor activities or meeting friends. We offer private and semi-private room options and have a limit of 48 residents. Rooms are spacious with modern touches. Bathrooms are all ADA-compliant with safety features throughout.
