The Reserve at Oswego (formally known as Autumn Leaves of Oswego) is a premier Memory Care and Assisted Living community in Oswego, IL, that serves the village, along with Montgomery, Plainfield, Naperville, Yorkville, Sandwich and Newark, and the surrounding regions. Whether your loved one requires some help with the activities of daily living (ADLs) or is living with the effects of dementia or Alzheimer’s disease, our dedicated staff takes great pride in delivering high-quality care that is compassionate, safe, and resident-centered. Our services are designed to meet our residents’ physical, emotional and spiritual needs 24 hours a day, seven days a week. We offer enriching activities, exciting social programs, wellness classes, restaurant-quality meals, and more.
Our skilled staff includes a Medical Director, an LPN, and a Certified Dementia Practitioner. We additionally have an HSD on the team who prepares individualized care plans for our residents and a nurse to administer all medications properly. Our experienced and compassionate staff are committed to the highest level of care.

We use tailor-fit assessment tools to evaluate each resident’s capabilities and strengths, so the program can best meet their specific needs. Executive Director
Sherri is the Executive Director at The Auberge at Brookfield. She has a BS in Health Administration. She is a licensed nursing home administrator and has been working in skilled communities for most of her career. She is also a Certified Dementia Practitioner and a Certified Montessori Dementia Care Professional. She has two daughters and three granddaughters.
